Making Space - The Program & All Workshops under Jen Pillipow 

 

Hypnotherapy Release 

I, the undersigned, hereby release Jen Pillipow from any liability or claims that could be made against her concerning my mental and/or physical well-being during the work that has been outlined and agreed upon (now and in the future) by filling out this form. By entering into an agreement for services with Jen Pillipow (the “Agreement”), I agree to be bound by the Terms and Conditions set out herein (the “Terms”).  

Scope of Practice

I understand that Jen Pillipow provides services such as hypnotherapy and Reiki (collectively, the “Services”) and that the Services at all times are subject to the Terms.

I understand that Jen Pillipow ​is not a licensed physician, psychologist, or medical practitioner of any kind and that hypnosis should not be considered a replacement for the advice and/or services, of a psychiatrist, psychologist, psychotherapist, or doctor. Your doctor or your healthcare provider knows you and your medical history. Consult your doctor or healthcare provider if you have any questions relating to a symptom or a medical condition.

Always:

• SEEK THE ADVICE OF YOUR DOCTOR OR HEALTHCARE PROVIDER WHENEVER YOU HAVE A PERSONAL QUESTION ABOUT A MEDICAL CONDITION OR SYMPTOM.

• NEVER DISREGARD PROFESSIONAL MEDICAL ADVICE, OR DELAY SEEKING MEDICAL ADVICE OR TREATMENT BECAUSE OF ANY CONTENT YOU READ OR LEARN ON THE SERVICES.

• CALL 911 (OR YOUR LOCAL EMERGENCY SERVICE) OR YOUR DOCTOR IMMEDIATELY IF YOU BELIEVE YOU MAY OR DO HAVE A MEDICAL EMERGENCY.

Participation and Guarantee 

I give Jen Pillipow​ full permission to hypnotize me and use tools from Rapid Transformational Therapy including memory regression and acknowledge that by committing to participating fully in the process by listening to the personalized recording provided to me for a period of twenty-one (21) days, I play an important role in my overall success. 

I understand that although hypnotherapy has an incredibly high success rate, ​Jen Pillipow​ cannot and does not guarantee results since my own personal success depends on many factors that ​Jen Pillipow has no ​control over, including my willingness and desire to affect the changes inside of myself. 

Confidentiality

Any information collected from you, including your contact details will be held in the strictest confidence. Your privacy will be respected. The details that you disclose are used solely for the purpose of providing you with the highest levels of service. No information will be passed on to any external parties without your express permission. Where express permission is given to share information with an external party, Jen Pillipow will only supply approved information that Jen Pillipow believes is in the best interests of you.

If it is necessary for Jen Pillipow to contact you, Jen Pillipow will use your preferred method e.g. mobile phone or email address. If Jen Pillipow needs to contact you at home or at work to change an appointment, Jen Pillipow will leave her name, number and state that the nature of her call is personal. If there is an emergency and Jen Pillipow needs to cancel the appointment at short notice, she will state this in her message. During your treatment period, please ensure that Jen Pillipow has your up-to-date contact details.

By agreeing to this form, I consent that Jen Pillipow​ may release information to a specific individual or agency if it has been determined that a child or elder is at risk of or is currently being abused; if I am in imminent danger to myself or others; or, if a subpoena of records has been requested. 

I understand that, at any time, the sharing of anonymous case histories is sometimes made to colleagues when seeking guidance and discussion. No personal details are ever used during these discussions.

Acknowledgements

I understand the risks involved with hypnotherapy and that results vary. I acknowledge the risks and continue with therapy at my own risk. I acknowledge that if I am bipolar, have suicidal ideation, or psychosis that I have obtained a doctor’s permission and I have disclosed this to Jen Pillipow. I acknowledge that I do not suffer from epilepsy and that I will be free from the influence of drugs or alcohol. I acknowledge that I have provided Jen Pillipow with the correct information including my address, phone number, and an emergency contact number. I acknowledge that if it’s a virtual session, the environment around me is safe and will remain distraction free during the course of the session with Jen Pillipow.

Limitation of Liability

IN NO EVENT SHALL JEN PILLIPOW BE LIABLE TO ME OR TO ANY THIRD PARTY FOR ANY LOSS OF USE, REVENUE OR PROFIT OR LOSS OF DATA OR DIMINUTION IN VALUE, OR FOR ANY CONSEQUENTIAL, INCIDENTAL, INDIRECT, EXEMPLARY, SPECIAL OR PUNITIVE DAMAGES WHETHER ARISING OUT OF BREACH OF CONTRACT, TORT (INCLUDING NEGLIGENCE) OR OTHERWISE, REGARDLESS OF WHETHER SUCH DAMAGE WAS FORESEEABLE AND WHETHER OR NOT SUCH PARTY HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES, AND NOTWITHSTANDING THE FAILURE OF ANY AGREED OR OTHER REMEDY OF ITS ESSENTIAL PURPOSE.

IN NO EVENT SHALL JEN PILLIPOW’S AGGREGATE LIABILITY ARISING OUT OF OR RELATED TO THIS AGREEMENT, WHETHER ARISING OUT OF OR RELATED TO BREACH OF CONTRACT, TORT (INCLUDING NEGLIGENCE) OR OTHERWISE, EXCEED THE AGGREGATE AMOUNTS PAID OR PAYABLE TO JEN PILLIPOW FOR THE SERVICES.

Force Majeure

I understand that Jen Pillipow shall not be liable or responsible to me, nor be deemed to have defaulted or breached this Agreement, for any failure or delay in fulfilling or performing any term of this Agreement when and to the extent such failure or delay is caused by or results from acts or circumstances beyond the reasonable control of Jen Pillipow including, without limitation, acts of God, flood, fire, earthquake, explosion, governmental actions, war, invasion or hostilities (whether war is declared or not), terrorist threats or acts, riot, or other civil unrest, national emergency, revolution, insurrection, epidemic, lock-outs, strikes or other labour disputes (whether or not relating to either party's workforce), or restraints or delays affecting carriers or inability or delay in obtaining supplies of adequate or suitable materials, materials or telecommunication breakdown or power outage.

Termination

Your permission to use the Services ends immediately if you violate any of clause or section of the Terms. Jen Pillipow has the sole discretion to suspend or terminate your access to or use of the Services without notification for any violation of these Terms, or for other conduct that Jen Pillipow deems unlawful or harmful to others. In the event of termination, you are no longer authorized to access the Services, you may not re-register for the Services, and Jen Pillipow will use whatever means necessary to enforce this termination.

Additional Provisions

The obligations of these Terms are separate and divisible, and in the event that any clause is deemed unenforceable, the balance of these Terms shall continue in full force and effect. These Terms constitute the full agreement between Jen Pillipow and I in connection with the Services and there are no other representations or warranties, express or implied, other than those set forth in these Terms. These Terms supersede all prior arrangements and understandings, if any, between Jen Pillipow and I regarding the Services. I may not assign my obligations under these Terms without the prior written consent of Jen Pillipow. Waivers of, and additions or changes to the contents of these Terms shall be binding only if in writing and signed by both parties. These Terms shall be governed by the laws of Saskatchewan and litigation regarding these Terms may only be brought before the Courts of the Province of Saskatchewan.

By signing below, I hereby agree that I have read, understood and accepted this Agreement and are hereby providing my consent to utilize Jen Pillipow’s services.

I expect Making Space participants to truly honour their investment, their mental health, and show up for themselves 100%. The content, hypnotherapy, live support within this online program are potent and can work when practiced within your own personal capacity, listening to and following your limits, while also opening up your curiosity for a new way of living.

As a Making Space alumni, you have lifetime access (it’s not literally your or my lifetime, but I hope we’ll be old together!).

Please note, this is a fairly new program and is subject to change in terms of offerings. I will always strive to provide the highest service and if a portion of the program offerings are not utilized I may opt to remove them and replace them with something more advantageous. This may mean moving or swapping monthly Q&A calls for open office hours or new content, depending on the needs of the group. I will poll the group to feel into what is the best for everyone.

Please don’t share your password with others. This is very disrespectful to me and to the work. Should we find you’ve been sharing your password, we will revoke access immediately with no refunds; you are expected to continue your payment plan. If you violate community rules and are asked to leave and withdrawn access, you are still expected to complete your payment plan.

There are NO REFUNDS on stand-alone workshops.

For students outside of Canada. I hereby agree that all aspects of the relationship between myself and Jen Pillipow the resolution of any and all disputes arising from or in connection with this relationship, including any disputes arising under or in connection with this Agreement, shall be governed by and construed in accordance with the laws of the Saskatchewan (other than conflict of laws rules) and the laws of Canada applicable therein.

I hereby acknowledge that the hypnotherapy and Reiki received will be provided from Saskatchewan, Canada, and that the Courts of Saskatchewan shall have exclusive jurisdiction to hear any complaint, demand, claim, proceeding or cause of action, whatsoever arising from or in connection with this program, or from any other aspect of the relationship between myself and Jen Pillipow.
